Physician supervision of a patient receiving Medicare-covered services requiring complex and multidisciplinary care involving regular physician involvement or revision of care plans, review of subsequent reports of patient status, review of diagnostic tests, and discussions with family. This must be 30 minutes or more within a calendar month. Documentation must be present in the patient medical record or log.
